Tasks
- Conducting social intake and maintaining individual case files
- Assessing and diagnosing needs and requests
- Developing an individual support and action plan
- Individual and group support for residents
- Designing and implementing support and empowerment activities
- Referrals to public services and other agencies
- Networking and collaboration with other departments or community organizations
